Assessment Form (M. Lata)

Assessment Form (M. Lata), otherwise misspelled as "Assessement Form" in-game, is a document found in Chapter 2: Fly in a Web of Poppy Playtime. It serves no purpose in completing the game, however, may be linked to a part of the lore. Obtained alongside its predecessor, they can both be found in the Statues Watchtower.

This assessment form is one of the two forms seen testing Makayla Hyssop. Within this assessment, Makayla preforms fairly better than her last test, however still lacks focus when playing through Statues. The author, that being Matteo Lata, claims a similar issue Makayla faced on a prior test featuring Sarah Abell.

Plot

Makayla Hyssop retakes her test after receiving a below-average score on her Game Station assessment test. She was and still is assigned Candy Cat by the Game Station protocolar Sarah Abell.

On her first trial, Matteo Lata would rate Makayla's efforts a 4/5 rating while playing Musical Memory, with Matteo stating that Makayla was very fast and accurate while making minimum mistakes in her playthrough.

On her second trial, Matteo Lata would rate Makayla's efforts a 3/5 rating while playing Wack-a-Wuggy. Similarly to her first assessment test, Matteo Lata explains that Makayala's accuracy and awareness to her surroundings need improvement. Her reaction timing is also stated to be fast.

On her third trial, Matteo Lata would rate Makayla's efforts yet another 4/5 rating while playing Statues, with Matteo stating that Makayla had completely lacked focus, and how unlike her other tests, she was not engaged with the test - instead had her attention drawn away from the obstacle. The document shortly cuts itself off after attempting to explain that this entity was bringing her attention away from the obstacle. An unintended pen line alongside a blood spatter are the only remains.

Theorizing

Because of the abrupt ending and bloodstains on this Assessment and that of Assessment Form (S. Abell), it is to suggest that the two writers were attacked by something and possibly killed. It alongside is unknown what "assigning a toy" to a person exactly means, however it implies with the nature of Playtime Co. that the victims assigned will later possess a toy. These toys can range from the average toy that can be seen in the Wuggies, or a large mascot like Huggy Wuggy.

Trivia

  • The form has several spelling mistakes. Assessment is spelled with an extra e, as 'Assessment', on this form as well as on the (S. Abell) form. Curiously, this is a misspelling of the printed text, whereas the later spelling mistakes could be attributed to the proctor making a careless mistake in their spelling, such as where spatial is misspelled as spatial.
